Specimen Collection and Packaging Instructions
 COLLECTION INSTRUCTIONS - FASTING IS NOT REQUIRED.
1. Collect two 8-ml blue/black speckled-top CPT tubes of blood
using a 21 gauge needle.
2. Gently invert 5 times to mix the blood thoroughly with the
anticoagulant.
3. DO NOT SPIN, REFRIGERATE or FREEZE the specimen.
4. Label tube with patient name and date. Affix sticker from
requisition.
5. Tubes and absorbent pad are placed back into clear plastic box.
6. The clear plastic box is put into the biohazard bag and sealed.
The completed requisition is put into the outside pocket.
7. The biohazard bag is then placed into the cardboard box.
8. Insert the box into the supplied Lab Pack envelope, seal and
affix the FedEx or DHL airbill to the indicated spot. (Up to
4 boxes may be placed in one Lab Pack envelope or may
combine with LPP boxes)
9. Complete the “Shipper” portion of the airbill.
10. For Fed Ex packages only, place the “Hold Saturday at Fed
Ex Location” sticker on the envelope.
SHIPPING INSTRUCTIONS (FED EX and DHL)
• DO NOT PLACE THE SHIPMENT IN A DROP BOX.
• DO NOT USE THE AUTOMATED SYSTEM.
• WHEN CALLING FOR A PICK UP SPEAK WITH A CUSTOMER
SERVICE REPRESENTATIVE.
• GIVE YOUR ADDRESS AS THE PICKUP LOCATION.
• It is very important that the lab receives the specimens
the day after collection.
• Call Fed Ex for courier pickup ASAP at 800-463-3339,
but no later than 3:00 PM, Monday – Friday.
• Call DHL for courier pickup at 800-225-5345.
The laboratory is open on Saturdays to receive specimens
collected on Fridays.
